2012-2013 Preseason Paperwork

Athlete Information Packet: This informational packet should be read and a copy retained for future reference by all new and returning Huntingdon College student-athletes.
Athlete and Parent Signature Packet: This packet needs to be completed by both new and returning Huntingdon College student-athletes.
Returning Athlete General Medical Physical Form: This packet should only be completed by those Huntingdon College student-athletes that completed an ortho physical during the 2012 spring semester. If you are a returner that did not complete a physical, then please complete the new/return packet listed below.
New Athlete and Returning Athletes without Spring Physical Medical History and Physical Form: This packet needs to be completed by any new Huntingdon College student-athlete and by any returning Huntingdon College student-athlete that did not complete a physical during the 2012 Spring Semester.
Drug Testing Consent Form: This form must be signed by all student-athletes and turned in on check-in day. Any student-athlete under the age of 20 must also have a parent or guardian's signature.
Incoming Student-Athlete Publicity Questionnaire: This form needs to be completed by all new Huntingdon College student-athletes. Returning student-athletes do not need to complete the form, unless there is a correction to be made or information needs to be updated.
